FIRST VISIT
Your initial appointment will typically consist of a comprehensive examination by one of our doctors, a periodontal (gum) evaluation by a dental hygienist, x-rays if appropriate, and a review of your treatment options and associated costs. If you are currently experiencing pain or a dental emergency, please contact the location nearest you, and we’ll schedule you for a problem-focused exam with one of our doctors to immediately address this concern.
We can’t wait to meet you, so to make your first visit as successful and easy as possible, please bring the following documents with you when you arrive at our office:
- Your insurance information so we can help estimate any out of pocket costs
- Any x-rays taken by a previous dentist, if applicable
- A list of medications you are presently taking
- A completed copy of our Medical History Form, Privacy Policy and Office Policy.
IMPORTANT: All patients under the age of 18 must be accompanied by a parent or guardian. Please alert the office if you have a medical condition that may be of concern prior to treatment (diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (heart medications, aspirin, anticoagulant therapy, medications for osteoporosis, etc.)

Commonwealth Dentistry is open Monday-Friday from 8 a.m.-5 p.m., but our hours can vary by location. Please call the office most convenient to you so we may schedule a time that best suits your needs:

We will schedule your appointment as promptly as possible. If you have pain or a dental emergency, every attempt will be made to see you on the day you call. Please arrive 10-15 minutes before your scheduled appointment to update any necessary paperwork or patient information.
DENTAL INSURANCE
Commonwealth Dentistry participates in many Insurance Plans, plus, we will gladly file claims with any dental insurance company. Please call us with any questions about our insurance policies.
We make every effort to provide you with exceptional care and outstanding customer service. That’s why we work hand-in-hand with you to inform you of the specifics of your dental insurance coverage, file claims on your behalf, and help you maximize your insurance reimbursement for covered procedures. If you have any questions concerning your dental insurance, please feel free to give us a call.
Please bring your insurance information with you to your initial appointment and anytime there is a change in your policy so we may be able to answer specific questions and access coverage information.

FINANCIAL POLICY
PATIENT BILLING
For your convenience, we accept Visa, MasterCard, and Discover. Unless other arrangements have been made in advance, we ask that your payment is due at the time service is rendered. If you have questions regarding your account, please feel free to contact us.
Please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage. We do send monthly account balance statements. We will file insurance claims on your behalf unless instructed otherwise. Insurance companies typically require 4-6 weeks to respond to claims. Please call our office if your statement does not reflect your insurance payment within that time frame. Any remaining balance after your insurance coverage payment is your responsibility. Your prompt remittance is appreciated. We can make arrangements for a monthly payment plan but this must be done in advance of procedures performed.

We value your privacy and our office privacy policy is available online or upon request at any of our offices.
